Getting your results
For most SimonMed exams, your results will be ready within 1-3 business days. This timeline may vary based on the type of scan or procedure. Your care team at SimonMed will let you know what you may expect during your visit. You will be able to view and share your results in your patient portal as soon as they are available. Additionally, as soon as results are available, they will be sent to your referring healthcare provider, or anyone else you requested to receive the results.

This depends on the type of exam. In many cases, your referring provider will discuss your results with you.
You may log into your patient portal to upload your previous exam records, or upload your records to our repository here.
In your patient portal, you can select any imaging results you wish to share with your care team and simply click “Share.” Enter the recipient’s email address to securely share your results.
For other avenues to request and share records, you may complete the Authorization to Release Protected Health Information form below and send the form via fax (#602-302-5958) or email (MedRecReq@simonmed.com) along with a photo copy of your valid ID. All requests will be expedited as quickly as possible (no longer than 30 days).
