Our hours of operation vary by site, and exam type. If you’re looking for a particular location, please visit our location page and find the site you need.
Make scheduling fast and easy with the following items:

Our SimonMed Longevity services have a dedicated team to help guide and schedule your appointments. Please give them a call to schedule your whole-body MRI or other Longevity screenings. Call 1-877-653-3558.
Yes, for most X-ray exams, you may be able to walk in. Please visit the Locations page to view the X-Ray walk-in hours for your location, and check the X-ray waiting queue to plan your visit.
However, X-ray Fluoroscopy exams are not done on a walk-in basis. These exams include UGI, Esophogram, UGI w/ SBFT, Barium Enemas, IVP, and HSG.

Login using your email ID and password. This should be one which you had set up at the time of enrollment. Once logged in, you will see your name on the top right corner. Next to it will be a drop down arrow, click on the drop down arrow and select Manage Appointments. Under Upcoming Appointments, you will see the option to Reschedule/Cancel your appointment. Please let us know as soon as possible if you need to change your appointment. Please review our No Show & Late Cancellation Policy for details.

Almost always, yes; you should receive an order from your provider prior to the exam. The exceptions to this include screening mammograms as well as SimonMed Longevity services. Other exam requirements may vary by state. Please give us a call for further details.
Self-referral is very limited as almost all exams require a provider order.
Most orders, including mammography orders, are good for one year.

Yes there is a portal, but some paperwork still could be required on-site depending on the exam.
Check-in time for most exams is 15 minutes prior to your appointment time. For MRI and Nuclear Medicine appointments check-in time is 30 minutes prior. CT with oral contrast is 45 minutes prior. Some specialty exams can require a check-in time of up to 60 minutes prior. When you schedule your exam we will share your specific check-in details.

SimonMed uses low dose protocols and the American College of Radiology guidelines. Most exams are less than exposure from long distance air travel.

Yes, please do not stop taking your medication unless you are specifically told by your physician to stop.

Spouse, family, and friends are welcome. If a lone parent or guardian is getting scanned, infants or toddlers should remain at home as we unfortunately do not have anyone to watch them and we cannot be responsible for their safety. If you have any further questions or accommodations need to be considered please don’t hesitate to call.
Patients under the age of 18 must be accompanied by an adult. You will need to fill out our parental consent to treat a minor form on arrival.
The majority of our scans and services do not require a driver. However, if you are taking any oral sedative or receiving our twilight sedation for your exam we do ask that a driver accompany you.
Yes, emotional support and service dogs can come to the center
Yes. We will simply need to see an alternate form of identification that has your picture on it.
Examples may include: auto registration, credit card in your name, a Costco or Sams Club card, a gym ID or school ID, a Health card, TAM card, or Sheriff’s card. If we have your ID on file from the previous visit we will also be able to use that.

Insurance & cost estimates
SimonMed accepts all major insurance plans: commercial, Medicare, Medicaid, workers’ compensation, and more. For questions regarding your insurance plan please call us and we’ll be happy to provide more information.
We do price match including schools for comparable technology.
In our experience most procedures receive same-day authorization. In rare instances, some insurance companies will require a review of the referring physician’s notes. This can take from 3 to 12 days.
SimonMed provides a pre-visit estimate based on your insurance. For those without insurance, we provide a cash price. In most cases, your out-of-pocket costs will be much lower than hospital-based imaging. For an estimated quote, please call or chat with a live agent.

SimonMed Imaging provides an estimated cost that will be due when you receive your physician ordered exam. SimonMed checks your insurance benefits, prior to services being rendered, to provide an estimated cost at time of service.

Based on the exam(s) ordered by your physician, your insurance company reports to SimonMed the estimated amount the patient may be responsible for based on the current status of their policy. This does not include any pending claims that have not been processed. Please note it takes 6-8 weeks for most claims to be processed by your insurance and to update deductible, coinsurance, and out-of-pocket maximum balances.

Understanding your results
Your patient portal is the best place to complete any required forms, view your exam results, and share your results with your healthcare team.
For most SimonMed exams, your results will be ready within 1-3 business days. This timeline may vary based on the type of scan or procedure. Additionally, as soon as results are available, they will be sent to your referring healthcare provider, or anyone else you requested to receive the results.
This depends on the type of exam. In many cases, your referring provider will discuss your results with you.
Paying your bill
Payment is expected at the time of service.

SimonMed accepts major credit cards, personal checks, or cash.
Yes, you most likely can pay with HSA/FSA funds. Please review your individual policy and ensure that your date of service is within the same calendar year as your payment.
Yes, we offer financing options through CareCredit. Please contact our billing department if you have any additional questions.
For SimonMed Longevity services: We accept financing from Klarna and Affirm in addition to CareCredit (Mastercard only).
A National Provider Identifier, or NPI, is issued to health care providers by Centers for Medicare and Medicaid. Tax ID is a tax identificaiton number used for tax purposes.

Medical records
The best way to securely share your exam records with your healthcare team is to log in to your patient portal. Through your portal, you may simply check the records you wish to share and enter your provider’s email address.
For any other requests, you may complete the Authorization to Release Protected Health Information form below and send the form via fax (#602-302-5958) or email (MedRecReq@simonmed.com) along with a photo copy of your valid ID. All requests will be expedited as quickly as possible (no longer than 30 days).
You may log into your patient portal to upload your previous exam records, or upload your records to our repository here.
For physical records requests, you may complete the Authorization to Release Protected Health Information form below and send the form via fax (#602-302-5958) or email (MedRecReq@simonmed.com) along with a photocopy of your valid ID. All requests will be expedited as quickly as possible (no longer than 30 days).
As a courtesy to patients, any requests for a CD of your medical images will be provided at no charge. For any patient requested films, a fee of $50.00 per set will be applied. If your referring physician specifically requests films, SimonMed will provide them at no charge.
Yes, as long as they are listed on your Authorization to Release PHI form and can show a valid photo ID.
You should be able to view, but IT support is available if there is difficulty loading the exam.
